Our Care Managers take an integrative approach to care coordination, providing comprehensive solutions tailored to your senior loved one’s individual needs, preferences and values. Partnering with your family, we create, implement and monitor a plan to address every aspect of aging, including:

Medical, Cognitive and Emotional Needs

Navigating the medical system & bureaucracy; referrals to health care professionals; coordinating and attending appointments; obtaining approvals and hitchayvuyot (guarantees) from the Kupot; scheduling and follow up of diagnostic testing; coordinating prescription delivery; obtaining equipment; communicating among health care professionals, family and caregivers.

Activities of Daily Living

Coordinating and supervising part-time or live-in care, including screening, training and supervising foreign caregivers; coordinating meal delivery or in-home food preparation; coordinating transportation; obtaining assistive devices such as GPS, toilet seats, walkers, wheelchairs, hospital beds; coordinating home maintenance (handyman), arranging laundry service, etc.

Home Safety

Conducting home safety and fall risk assessments and implementing modifications and technology; installation of surveillance cameras and/or technology to monitor in-home care

Maximizing Benefits

Identifying all potential benefits, including Bituach Leumi, Kupat Cholim, private insurance coverage, Holocaust survivor benefits, senior citizen discounts, etc.; handling all paperwork and follow-up.

Legal & Financial Matters

Help think about decision-making in the event of incapacity; referring to a vetted family lawyer to set up the right legal documents (power of attorney, advance directive, guardianship); helping manage household finances and bills and looking out for potential financial abuse and scams.

Social Interaction

Recommending and coordinating social, recreational, and intellectual activities to combat loneliness and promote physical and emotional well-being, such as lectures, day centers, home visits, and volunteering,

Communication & Conflict Resolution

Maintaining regular contact with family members and care providers; facilitating dialogue between aging parents and adult children; helping siblings make decisions.

Advocating in times of hospitalization, health crisis or chronic illness and treatment path, to ensure our clients get the best care possible.

A Day in the Life of a Care Manager

HOME VISIT

Weekly home visit with Client; looking at new photos of grandchildren and helping set up iPad; discussing daily routine with caregiver to include cognitive exercises; scheduling home visit by podiatrist; reviewing caregiver log.

DOCTOR VISIT

Accompanying client to doctor appointment; scheduling MRI and appointment with neurologist; picking up prescription medication and pill organizer; conveying follow-up instructions to caregiver and updated family.

PATIENT ADVOCACY

Meeting client at ER after fall and advocating for timely attention and appropriate testing; coordinating private night caregiver and waiting for her arrival; maintaining ongoing communication with caregiver and family.

CAREGIVERS

Interviewing 4 potential foreign caregivers for live-in position; checking references and scheduling time for Client and daughter to interview; sending list of interview questions to daughter; checking status of Permit with Misrad Ha’Pnim.

HOUSING

Touring English speaking assisted living facilities in Jerusalem with Client for father making Aliyah; speaking with NBN regarding flight arrangements.

ASSESSMENT & PLAN

Meeting with new Client at her house to begin assessment and plan for care; conducting home safety review and coordinating delivery of raised toilet seat and walker from Yad Sarah.

BACK OFFICE

Obtaining online code for Bituach Leumi for Client and submitted application for Gimlat Siyud; speaking with hospital social worker to coordinate discharge and rehabilitation; coordinating Client’s enrollment to Melabev Day Center including transportation and meals; meeting with family on Zoom for updates and discussing Care Plan.
