ONGOING CARE MANAGEMENT

By your side, wherever life takes you

An ongoing relationship with your family and routine home visits with your aging loved one enables us to be proactive and "hands-on" - providing the right amount of assistance at the right time to ensure well-being, maximum quality of life and peace of mind.

Hands-on care that's always one step ahead

Our Care Managers take the time to understand and embrace your loved one's individual needs, preferences and values. A long-term plan of comprehensive care coordination alongside meaningful communication with family and professionals ensures they receive quality care to support their physical, emotional and social well-being.

When do I need ongoing care management?

Whether you live far away, are overwhelmed trying to juggle the demands of family and work, or just want the very best for your aging loved one, our Care Managers are your eyes, ears and feet on the ground in Israel.

Routine home visits enables your Care Manager to:

Promote individualized care to meet your loved one's unique needs and preferences.

Monitor quality of care, advocate on your loved one's behalf, and prevent neglect and abuse.

Identify problems, present solutions, facilitate communication and assist with decision-making.

Anticipate issues and address them before they rise to the level of crisis.

Maintain a personal connection and provide emotional support and social engagement.
