Position:  Geriatric Care Manager, Jerusalem

Scope:  Part-time (20 hours per week) to Full-time (40 hours per week); Flexible Hours

Compensation:  Hourly, commensurate with experience

Care Manager Duties

  • Work with families to identify present and future care needs.
  • Create and implement a personalized care plan.
  • Conduct home visits, routine and as needed.
  • Coordinate care ongoing, including coordinating medical and paramedical care, hiring/supervising caregivers, applying for benefits, etc.
  • Liaison with family and assist in facilitating decision-making.
  • Manage crisis situations.
  • Advocacy!

Requirements

  • Academic degree in social work, gerontology, nursing, psychology or related fields.
  • 2+ years professional experience in Israel in an aging-related field.
  • Proficiency in both English and Hebrew.

  • Is comfortable using cloud-based applications and programs.
