B’Lev Shalem: Personalized Support for Aging Well
B’Lev Shalem (formerly known as ElderOptions Israel) introduces to Israel the American model of “geriatric care management,” a profession that emerged in the U.S. in the 1980s in response to a recognized gap in aging services: the population of older adults was growing rapidly and their needs for the community and health care services that would allow them to remain in their homes were not being met.
Many adult children had moved far away from their older parents, while others were strained raising their own families and working as dual earners. Geriatric Care Management answered both the needs of older adults and their families by tying together public, private and non-profit programs and entitlements that affect older adults, and by filling the gaps in available services. Today’s Geriatric Care Managers focus their practices on planning and coordinating the care of older adults across a wide spectrum of needs including medical, financial, social, housing, family and personal.
Our Care Managers are experts at understanding needs and developing a plan of action; they know what what resources are out there, and how to take advantage of them.
Recognizing a dire need for organized Care Management in Israel, Stacey Gordon, founder of ElderOptions NY/Chicago, and Amos Gil brought the U.S. model of Care Management to Israel in 2012 when they founded ElderOptions Israel. Today B’Lev Shalem provides older adults across Israel and their families around world with high quality Care Management Services based on best U.S. clinical standards and practice, creating a new standard of care for Israel’s older population.
